In a bold move to reduce one of the world\u2019s deadliest childbirth complications, leading global health organizations today released groundbreaking new guidelines for the prevention, diagnosis, and treatment of postpartum haemorrhage (PPH). The comprehensive recommendations\u2014unveiled at the 2025 FIGO World Congress<\/strong>\u2014are expected to significantly shift how maternal health providers respond to this life-threatening emergency, especially in low-resource settings.<\/p>\n

Jointly published by the World Health Organization (WHO)<\/strong>, the International Federation of Gynecology and Obstetrics (FIGO)<\/strong>, and the International Confederation of Midwives (ICM)<\/strong>, the guidelines call for earlier detection and faster intervention<\/strong> in cases of PPH, which kills nearly 45,000 women<\/strong> every year and affects millions more<\/strong> with lasting health consequences.<\/p>\n

\u201cPostpartum haemorrhage is the most dangerous childbirth complication since it can escalate with such alarming speed,\u201d said Dr Jeremy Farrar<\/strong>, WHO\u2019s Assistant Director-General for Health Promotion and Disease Prevention and Care. \u201cWhile it is not always predictable, deaths are preventable with the right care. These guidelines are designed to maximize impact where the burden is highest and resources are most limited.\u201d<\/p><\/blockquote>\n

\n

PPH: A Silent Crisis in Maternal Health<\/strong><\/h3>\n

Postpartum haemorrhage\u2014defined as excessive bleeding after childbirth\u2014is the leading cause of maternal mortality globally<\/strong>. It can progress rapidly and unpredictably, often without prior risk factors. When not fatal, PPH can lead to severe complications including organ failure, infertility, and psychological trauma.<\/p>\n

The new guidelines are based on the largest-ever study on PPH<\/strong>, published today in The Lancet<\/em>, and they introduce a more sensitive diagnostic threshold<\/strong> to trigger early response. While traditionally diagnosed at 500 mL of blood loss<\/strong>, health workers are now advised to initiate treatment at 300 mL<\/strong>, especially when accompanied by abnormal vital signs<\/strong>.<\/p>\n

\n

Objective Diagnosis with Simple Tools<\/strong><\/h3>\n

To support early diagnosis, the guidelines advocate for the use of calibrated drapes<\/strong>\u2014low-cost, simple devices that collect and measure blood loss in real time. These tools are essential in busy or under-resourced labour wards where subjective estimations often delay treatment.<\/p>\n

\n

The MOTIVE Bundle: A Rapid-Response Protocol<\/strong><\/h3>\n

Once PPH is diagnosed, the guidelines recommend immediate activation of the MOTIVE bundle<\/strong>, a structured set of actions designed to stop bleeding and stabilize the mother:<\/p>\n

    \n
  • M<\/strong>assage of the uterus<\/li>\n
  • O<\/strong>xytocic drugs to stimulate contractions<\/li>\n
  • T<\/strong>ranexamic acid (TXA) to reduce bleeding<\/li>\n
  • I<\/strong>ntravenous fluids<\/li>\n
  • V<\/strong>aginal and genital tract examination<\/li>\n
  • E<\/strong>scalation of care if bleeding continues<\/li>\n<\/ul>\n

    If bleeding persists, interventions such as surgery or blood transfusion<\/strong> should be implemented as emergency stabilizing measures.<\/p>\n

    \u201cWomen affected by PPH need care that is fast, feasible, effective and drives progress towards eliminating PPH-related deaths,\u201d said Professor Anne Beatrice Kihara<\/strong>, President of FIGO. \u201cThese guidelines take a proactive approach of readiness, recognition and response.\u201d<\/p><\/blockquote>\n

    \n

    Focus on Prevention and Antenatal Support<\/strong><\/h3>\n

    The guidelines also place strong emphasis on prevention<\/strong>, particularly the management of anaemia<\/strong>\u2014a common condition in pregnant women that worsens PPH outcomes. Recommendations include:<\/p>\n

      \n
    • Daily oral iron and folate supplements<\/strong> during pregnancy<\/li>\n
    • Intravenous iron therapy<\/strong> in cases of severe anaemia or after PPH<\/li>\n
    • Discouraging unsafe practices like routine episiotomies<\/strong><\/li>\n
    • Encouraging perineal massage<\/strong> in late pregnancy to reduce trauma during childbirth<\/li>\n<\/ul>\n

      During the third stage of labour<\/strong>, the administration of a uterotonic drug<\/strong> remains crucial. Preferred options include oxytocin<\/strong> or heat-stable carbetocin<\/strong>. If these are unavailable and the cold chain is unreliable, misoprostol<\/strong> may be used as a last resort.<\/p>\n

      \n

      A Global Call to Action<\/strong><\/h3>\n

      The release of these guidelines comes with a strong appeal to governments, donors, and health systems<\/strong> to invest in maternal care and frontline workers.<\/p>\n

      \u201cMidwives know first-hand how quickly postpartum haemorrhage can escalate and cost lives,\u201d said Professor Jacqueline Dunkley-Bent OBE<\/strong>, Chief Midwife of ICM. \u201cThese guidelines are a game-changer. But to end preventable deaths from PPH, we need more than evidence and protocols. We need commitment, funding, and action.\u201d<\/p><\/blockquote>\n

      The launch is supported by a suite of training materials and implementation tools<\/strong>, including:<\/p>\n

      • Hands-on modules for frontline providers<\/li>\n
      • National-level integration strategies<\/li>\n

        A Major Milestone in Global Maternal Health<\/strong><\/h3>\n

        This marks the first consolidated international guideline focused solely on PPH<\/strong>, forming a cornerstone of the Global Roadmap to Combat Postpartum Haemorrhage (2023\u20132030)<\/strong>. The initiative aims to eliminate preventable maternal deaths due to PPH over the next five years.<\/p>\n
