In a landmark achievement for minimally invasive gynaecological surgery, doctors at Mandav Hospital, Gutkar in Mandi district of Himachal Pradesh, successfully removed a massive uterine fibroid weighing 5.21 kilograms through a purely laparoscopic procedure—one of the largest such cases ever reported in India.

The patient, a woman who had endured years of progressive abdominal distension, heavy menstrual bleeding, and severe pressure-related symptoms, was found to have a uterine fibroid comparable in size to a full-term pregnancy. The condition had significantly impaired her quality of life, making surgical intervention unavoidable.

Given the extraordinary size of the uterus and the complexity associated with such cases, the surgery posed considerable challenges. After comprehensive clinical evaluation, detailed pre-operative planning, and careful risk assessment, the medical team elected to pursue a minimally invasive laparoscopic approach—despite the fact that fibroid uteri of this magnitude are traditionally managed through open surgery.

The complex procedure was performed by Dr. Uday Bhanu Rana, Consultant Gynaecologist and Advanced Laparoscopic Surgeon, and lasted approximately 2.5 to 3 hours. The operation was described as technically demanding due to the uterus’s massive size and weight, severely distorted pelvic anatomy, limited operative space, and markedly increased blood supply, all of which significantly elevate surgical risk.

To overcome these challenges, the team employed advanced 3D laparoscopic vision, meticulous step-by-step dissection, and systematic devascularisation techniques to ensure safety and precision. Despite the high-risk nature of the surgery, it was completed successfully without any intra-operative or post-operative complications.

According to available medical literature and previously documented cases, the largest fibroid uterus removed laparoscopically in India prior to this weighed approximately 4.5 kg. The successful removal of a 5.21-kg uterus through a minimally invasive approach places this case among the largest ever managed laparoscopically, marking a major milestone in the evolution of gynaecological surgery.

Dr. Rana was supported by a skilled multidisciplinary team that included Dr. Priyanka Sharma (Fellow Gynaecologist), Dr. Anshit Pathania (Anaesthetist), scrub nurses Shivani Sharma and Harshita Sharma, and OT assistants Mahesh and Jatin.
